Maple Hill Families,
This is a reminder about the health requirements for starting school in September 2023.
Physicals
All students entering grades 7, 9 and 11 are still required to submit updated physicals dated within a year before the start of school on September 7. Students will have until the end of September to submit the required health records. If more time is needed, please let me know. If your child is planning to play a fall sport, an updated physical dated within a year before their sport starts is also required for participation.
Immunizations
All students must be up to date with immunizations within 14 days of the start of school. Please call your child’s physician to see if your child needs any immunizations. Every new student entering school needs to submit an accurate immunization record.
Every student entering 7th grade must submit proof of Meningococcal vaccination, regardless of age.
Every student entering 12th grade must submit proof of a second Meningococcal vaccine unless the first dose was received at 16 years of age or older.
